I'm treating my 2 feeder goldfish for ich.  Because they also had fin and tail rot, I first got the tank stabilized (there were carbonate hardness issues), then treated with Maracyn Two and Maracide for the ich, so as not to mix treatments from different manufacturers. 

But the Maracide claims to treat the fish, not the water.  I don't understand this, because from what I'm reading here on Petfish, it's the ich in the water that should be treated, not the ich on the fish.  Am I correct?

My fish have just finished their 5 day course of treatment.  I can still see ich spots.  Does this mean I should treat again?  Should I give them a rest and then treat with a different ich medicine, since they are done with the Maracyn Two course?  Or should I just concentrate on clean/balanced tank water, gravel vacuuming and salt for awhile and see what happens?

Since treating the ich that is *not* on the fish is the goal, which medicines are best for that?  the Maracide contains Malachite Green and chitosan.
Will it work?  Will it work in the 5 days recommended? Is it safe to redose?  (The bottle doesn't say).

You are right about Ich on the fish body, nothing will touch the white spot but,
The medication should contain both Malachite green AND formalin. If it doesn't have both, find another bottle of another manufacturer.
